Tasakaalu hindamine"},"content":{"rendered":"<div>\n<div id=\"_com_1\">\n<h6>\n\t\t\tTasakaal on v\u00f5ime s\u00e4ilitada keha raskuskese tugipinna kohal.<br>\n\t\t<\/h6>\n<\/div>\n<\/div>\n<p style=\"margin-top:12.0pt;margin-right:0cm;margin-bottom:12.0pt;margin-left:0cm\">\n\t<span lang=\"et\" style=\",serif\">Tasakaal ehk posturaalne stabiilsus on v\u00f5ime s\u00e4ilitada keha raskuskese (<i>center of mass<\/i>) tugipinna\u00a0kohal. Normaalse posturaalse stabiilsuse korral toimub keha pidev k\u00f5ikumine (<i>sway<\/i>) ehk keha raskuskeskme nihkumine tugipinna suhtes. Inimene s\u00e4ilitab tasakaalu kui keha k\u00f5ikumine p\u00fcsib stabiilsuse piirides (<i>limits of stability<\/i>). Tasakaalu s\u00e4ilitamisel on tavaline kindlas ulatuses keha anteroposterioorne ja mediolateraalne k\u00f5ikumine. T\u00e4iskasvanu normaalne anteroposterioorse k\u00f5ikumise vahemik on 12\u00ba. Mediolateraalne k\u00f5ikumine s\u00f5ltub tugipinna laiusest ja inimese pikkusest, kuid keha k\u00fclgsuunalise k\u00f5ikumise keskmine vahemik on 16\u00ba. Kui keha \u00fclem\u00e4\u00e4rasel k\u00f5ikumisel pole vaja muuta tugipinda, kasutab inimene tasakaalu s\u00e4ilitamiseks kohastrateegiat (<i>in-placed strategy<\/i>), kui keha k\u00f5ikumine \u00fcletab stabiilsuse piirid, kasutatakse tugipinna muutumise strateegiat (<i>change-in-support<\/i>) uue tugipinna leidmiseks. M\u00f5lemad strateegiad tulenevad keskn\u00e4rvis\u00fcsteemi poolt koordineeritud tasakaalufunktsioonidest.<\/span>\n<\/p>\n<h6>\n\tNormaalsel juhul s\u00e4ilitab keha stabiilse asendi nii liigutustegevuse eel, ajal kui p\u00e4rast.<br>\n<\/h6>\n<p style=\"margin-top:12.0pt;margin-right:0cm;margin-bottom:12.0pt;margin-left:0cm\">\n\t<span lang=\"et\" style=\",serif\">Tasakaal t\u00e4hendab v\u00f5imet s\u00e4ilitada stabiilne kehaasend nii enne liigutustegevust, selle ajal kui\u00a0<\/span><span lang=\"et\" style=\",serif\">vahetult p\u00e4rast tahtlikku liigutust, aga ka suutlikkust reageerida v\u00e4listele \u00e4rritajatele. Indiviidi v\u00f5ime s\u00e4ilitada tasakaal k\u00f5ikumisega seotud stabiilsuse piirides s\u00f5ltub keha raskuskeskme asendi kontrollist tugipinna suhtes. N\u00e4iteks on Parkinsoni diagnoosiga v\u00f5i osteoporoosi tagaj\u00e4rjel sekundaarselt tekkinud rinnak\u00fcfoosiga patsiendil anterioorse kehahoiu t\u00f5ttu tasakaalu s\u00e4ilitamisel v\u00e4iksem vastupanuv\u00f5ime anterioorse k\u00f5ikumise suhtes. Ka parema p\u00f5lveliigese endoproteesimise v\u00f5i vasaku ajupoole insuldi j\u00e4rgselt v\u00f5ivad patsiendid seista nii, et nende keha raskuskese on enam nihkunud vasaku alaj\u00e4seme suunas ning nad pole v\u00f5imelised kandma t\u00e4iskeharaskust paremale alaj\u00e4semele. Kirjeldatud n\u00e4idete korral v\u00f5ib keha raskuskeskme lateraalsuunaline nihkumine suurendada kukkumisriski vasakule.<\/span>\n<\/p>\n<div>\n<div id=\"_com_3\">\n<h6>\n\t\t\tTasakaal s\u00f5ltub skeleti-lihass\u00fcsteemi funktsionaalsusest, sensoorsest tagasisidemehanismist, enesetajust, kognitiivsetest ning kardiopulmonaalsetest v\u00f5imetest.<br>\n\t\t<\/h6>\n<\/div>\n<\/div>\n<p style=\"margin-top:12.0pt;margin-right:0cm;margin-bottom:12.0pt;margin-left:0cm\">\n\t<span lang=\"et\" style=\",serif\">Tasakaal ei s\u00f5ltu ainult keha raskuskeskme paiknemisest tugipinnal, vaid ka lihaste ja liigeste ning n\u00e4rvide funktsionaalsusest, tugipinna suurusest, sensoorsest tagasisidemehanismist, enesetajust ehk ruumis orienteerumisv\u00f5imest, kognitiivsetest protsessidest, emotsioonidest ning kardiopulmonaalsest v\u00f5imekusest. Kahjustus mistahes nimetatud s\u00fcsteemis viib tasakaalu- ja funktsioonih\u00e4ireteni. Inimestel, kes on altid kukkuma, on v\u00e4iksemad keha k\u00f5ikumise stabiilsuse piirid. Alaj\u00e4seme lihaste j\u00f5u langus, puudulik lihaselastsus v\u00f5i liigeste liikuvuse piiratus tekitavad liikumis- ja tasakaaluh\u00e4ireid. Primaarsetele funktsionaalsetele h\u00e4iretele v\u00f5ivad lisanduda sekundaarsed h\u00e4ired \u2013 n\u00e4iteks vestibulaaraparaadi d\u00fcsfunktsioon v\u00f5ib kujuneda patsiendil, kes kurdab kaelavalu ja j\u00e4ikust kaela- \u00f5lav\u00f6\u00f6tme piirkonnas.<\/span>\n<\/p>\n<p style=\"margin-top:12.0pt;margin-right:0cm;margin-bottom:12.0pt;margin-left:0cm\">\n\t<span lang=\"et\" style=\",serif\">Patsiendi detailne hindamine v\u00f5imaldab tuvastada probleemse s\u00fcsteemi ning tagada kvaliteetse ravi. Kvaliteetne ravi s\u00f5ltub ka terapeudi oskusest seada prioriteediks sekkumised, mis l\u00e4htuvad p\u00f5hikahjustuse m\u00f5just erinevatele tasakaalu m\u00f5jutavatele s\u00fcsteemidele nii, et patsiendil oleks v\u00f5imalikult v\u00e4he tegevus- v\u00f5i osaluspiiranguid. <\/span>\n<\/p>\n<h6>\n\tNeljaastmelises tasakaalutestis seistakse labajalad koos, \u00fche labajala keskosa vastu teise labajala suurt varvast, tandmeseisus ja \u00fchel jalal.<br>\n<\/h6>\n<h4>\n\tNELJAASTMELINE TASAKAALUTEST<br>\n<\/h4>\n<p style=\"margin-top:12.0pt;margin-right:0cm;margin-bottom:12.0pt;margin-left:0cm\">\n\t<span lang=\"et\" style=\",serif\">Neljaastmeline, staatilise tasakaalu test hindab vanemaealise patsiendi kukkumisriski ning v\u00f5imet s\u00e4ilitada stabiilne kehaasend neljas, progresseeruvat v\u00e4ljakutset esitavas asendis. Testi ajal seisab terapeut patsiendi k\u00f5rval ning on valmis tasakaalu kaotuse korral patsienti toetama. Testi ajal on patsiendi silmad avatud. Kui patsiendi asend on testi eel stabiilne, k\u00e4ivitab terapeut stopperi. Kui patsient suudab hoida asendit 10 sekundit ilma jalgu liigutamata ega tuge vajamata, j\u00e4tkatakse testi j\u00e4rgmises asendis. Kui patsient pole v\u00f5imeline testiasendit 10 sekundi jooksul s\u00e4ilitama, siis test l\u00f5petatakse. Testi ajal ei tohi patsient kasutada k\u00e4imisabivahendeid. Patsiendi kukkumisriski n\u00e4itab suutmatus p\u00fcsida 10 sekundit tandemseisus (\u00fclesanne 3).<\/span>\n<\/p>\n<h4>\n\tFUNKTSIONAALNE HAARDEULATUSE TEST<br>\n<\/h4>\n<div>\n<div id=\"_com_5\">\n<h6>\n\t\t\tFunktsionaalne haardeulatuse test hindab vanemaealiste d\u00fcnaamilist tasakaalu ning kukkumisriski.<br>\n\t\t<\/h6>\n<\/div>\n<\/div>\n<p style=\"margin-top:12.0pt;margin-right:0cm;margin-bottom:12.0pt;margin-left:0cm\">\n\t<span lang=\"et\" style=\",serif\">Funktsionaalne haardeulatuse test (<i>functional reach test<\/i>) hindab vanemaealiste inimeste d\u00fcnaamilist tasakaalu, mille tulemus korreleerub kukkumisriskiga. Enne testi sooritamist kinnitab terapeut seinale 1,2- 1,3m k\u00f5rgusele m\u00f5\u00f5dulindi v\u00f5i muu m\u00f5\u00f5tmist v\u00f5imaldava vahendi. Testi eel seisab patsient ilma seinale toetamata, seina \u00e4\u00e4res, \u00fclaj\u00e4se \u00f5laliigesest painutatud 90\u00b0 ning s\u00f5rmed rusikas nii, et 3. metakarpaalluu on kohakuti m\u00f5\u00f5dulindi nullpunktiga (joonis 2A). Terapeut juhendab patsienti painutama ette nii kaugele kui v\u00f5imalik, kuid nii, et s\u00e4iliks kindel tugipind; m\u00f5\u00f5tmistulemust n\u00e4itab 3. metakarpaalluu asukoht m\u00f5\u00f5dulindil (joonis 2B). Test katkestatakse kui patsient astub sammu ette v\u00f5i kaotab tasakaalu. Funktsionaalne haardeulatuse test sooritatakse kolm korda, testitulemusena fikseeritakse kahe viimase testi keskmine. Bergi tasakaalutest koosneb 14. \u00fclesandest, millest iga <\/span><span lang=\"et\" style=\",serif\">hinnatakse viiepalliskaalal punktidega 0-4, sealjuures 0 n\u00e4itab madalaimat ning 4 k\u00f5rgeimat funktsioonitaset. Testi sooritamiseks vajalikud vahendid on joonlaud, 1 k\u00e4etugedega tool, 1 k\u00e4etugedeta tool, step-pink, stopper ja 5m k\u00f5nniruumi. Testi sooritamiseks kulub aega keskmiselt 20 minutit.<\/span>\n<\/p>\n<p style=\"margin-top:12.0pt\">\n\t<b><span lang=\"et\" style=\",serif\">1.
