{"id":16,"date":"2024-04-03T23:11:00","date_gmt":"2024-04-03T20:11:00","guid":{"rendered":"https:\/\/sisu.ut.ee\/antikehad\/6-terapeutilised-antikehad-immuunhaiguste-ja-muude-haiguste-ravis\/"},"modified":"2024-04-03T23:11:04","modified_gmt":"2024-04-03T20:11:04","slug":"6-terapeutilised-antikehad-immuunhaiguste-ja-muude-haiguste-ravis","status":"publish","type":"page","link":"https:\/\/sisu.ut.ee\/antikehad\/6-terapeutilised-antikehad-immuunhaiguste-ja-muude-haiguste-ravis\/","title":{"rendered":"4.2 Terapeutilised antikehad immuunhaiguste ja muude haiguste ravis"},"content":{"rendered":"<p style=\"text-align: left\">Immuunhaiguste ravis antikehadega on peamisteks ravisihtm\u00e4rkideks:<\/p>\n<ul>\n<li>ts\u00fctokiinid, immuunefektormolekulid (komplement, IgE);<\/li>\n<li>immuunrakkude pinnaretseptorid;<\/li>\n<\/ul>\n<p>Antikehade peamisteks toimemehhanismideks immuunhaiguste korral on:<\/p>\n<ul>\n<li>p\u00f5letikuts\u00fctokiinide retseptorile seondumise blokeerimine;<\/li>\n<li>immuunrakkude retseptorite blokeerimine v\u00f5i allareguleerimine;<\/li>\n<li>immuunrakkude eemaldamine l\u00e4bi antikeha efektormehhanismide;<\/li>\n<\/ul>\n<p>P\u00f5letikulised ja autoimmuunhaigused on kasvajate k\u00f5rval suurim haiguste grupp terapeutiliste antikehade kasutamiseks. Immuunrakkudel ja ts\u00fctokiinidel on keskne roll immuunhaiguste patogeneesis. Seet\u00f5ttu on neile suunatud terapeutiliste antikehade kasutamine immuunhaiguste ravis olnud tulemuslik.<\/p>\n<p>Heaks n\u00e4iteks on TNF-\u03b1 funktsiooni blokeerivad antikehad p\u00f5letikuliste soolehaiguste ja reumatoidartriidi ravis, andes h\u00e4id ravitulemusi ja olles ka suurimate m\u00fc\u00fcginumbritega terapeutilised antikehad. \u00a0TNF-\u03b1 on sihtm\u00e4rgiks neljale registreeritud terapeutilisele antikehale, milledeks on kim\u00e4\u00e4rne antikeha (Infliximab), inimese antikehad saaduna faagkuva meetodil (Adalimumab) v\u00f5i transgeenses hiires (Golimumab) ning humaniseeritud ja peg\u00fcleeritud\u00a0 Fab fragment (Certolizumab pegol). Kuigi sama sihtm\u00e4rgi vastu, on erinevused TNF-\u03b1 vastaste terapeutiliste antikehade toimemehhanismides, immunogeensuses, manustamisviisis ja ravitoimetes.<\/p>\n<p>Levinud nahahaiguse psoriaasi ravis on saadud h\u00e4id tulemusi interleukiin 12 ja 23 \u00fchise sub\u00fchiku vastase terapeutilise antikehaga Ustekinumab (inimese IgG<sub>1<\/sub>), mida manustatakse nahaaluste s\u00fcstetena.<\/p>\n<p>Kolmanda faasi kliiniliste uuringute j\u00e4rgus on hulk nii seniste kui uute sihtm\u00e4rkide (IL-5, IL-6, IL-13, IL17a, CD3, CD6, CD22) vastaseid terapeutilisi antikehi immuunhaiguste raviks ning nimekiri on palju pikem varasemas uurimisj\u00e4rgus olevate ravimkandidaatide osas.<\/p>\n<p style=\"text-align: center\">Tabel 3. Infektsioonhaiguste raviks kasutatavad kaks antikeha on m\u00f5lemad kasutusel prof\u00fclaktikaks: Palivizumab respiratoor-s\u00fcnts\u00fctiaalviiruse nakkuse ennetamiseks v\u00e4ikelastel ja Raxibacumab inhaleeritava katku ennetamiseks ja raviks.
